The site offers metrics updated every six months of the quality of care and of health outcomes for common chronic conditions such as congestive heart failure and diabetes.

Commission's Connected Cleveland 2020 Citywide Plan seeks to create connections between people and places and opportunities. Health impact is at heart of Commission's effort, leading to high utilization of socioeconomic and health data. One of the biggest current opportunities is collaboration with MetroHealth campus transformation project to ensure integration and thriving of surrounding communities.

Cuyahoga County Planning Commission
The mission of Cuyahoga County Planning Commission is to inform and provide services in support of the short and long term comprehensive planning, quality of life, environment, and economic development of Cuyahoga County and its cities, villages and townships. The Commission serves as a planning and decision-making resource for the public sector; agency staff provide services to the County, municipalities, regional agencies and not-for-profit organizations. The Commission provides rich socioeconomic, development and environmental data. Census Affiliate link on the website provides updated census profiles for area municipalities, Cuyahoga County, and adjacent counties. The site also includes the full Cuyahoga County Urban Tree Canopy Assessment (2013). A new interactive "Greenprint Viewer" will become available in June 2015.

Neighborhood Environmental Assessment Project
The Neighborhood Environmental Assessment Project (NEAP) is a systematic environmental mapping initiative. Each year, the CWRU Prevention Research Center for Healthy Neighborhoods team collects valuable community asset data to better understand the physical (or built) environment, such as food retail locations. These data are used to create static maps or searchable, interactive maps to give researchers and community members a better understanding of the physical (or built) environment and how that affects the health of residents.
NEOCANDO - Northeast Ohio Community and Neighborhood Data for Organizing
NEOCANDO is the Northeast Ohio Community and Neighborhood Data for Organizing offered by Case Western Reserve University’s Mandel School of Applied Social Sciences. NEO CANDO provides detailed social and economic data for 17 counties in Northeast Ohio plus neighborhood level data on certain topics.

Northeast Ohio Areawide Coordinating Agency (NOACA) Maps and Data (GIS portal)
The Northeast Ohio Areawide Coordinating Agency (NOACA) is a transportation and environmental planning agency serving Cuyahoga, Geauga, Lake, Lorain and Medina counties. NOACA is the federally designated metropolitan planning organization for northeast Ohio, and is charged with determining which proposed highway, bikeway and transit projects will receive federal funding. NOACA’s Transportation System Data Portal allows users to access NOACA transportation system data in a clickable GIS map-based environment.
Northern Ohio Data & Information Service (NODIS)
The Northern Ohio Data and Information Service (NODIS) is part of the Maxine Goodman Levin College of Urban Affairs, Cleveland State University. NODIS is designated by the State of Ohio and the U.S. Bureau of the Census as the regional data center for northern Ohio. NODIS is a depository for U.S. Census information, which is made available to the university community, public administrators, civic organizations and the general public on a cost-recovery basis.
PRCHN - Prevention Research Center for Healthy Neighborhoods
The mission of the Prevention Research Center at Case Western Reserve University is to foster partnerships within urban neighborhoods to develop, test, and implement effective, scalable, and sustainable strategies to prevent and reduce the burden of chronic disease. PRCHN projects reflect this mission through their involvement of neighborhood residents, leaders, and community organizations with the goal of addressing significant environmental and lifestyle issues that serve as barriers to good health.
