Community Health Status Assessment

This report describes who we are and where live (demographics), our education and poverty levels (socioeconomic status), access to healthcare, health‚Äźrelated behaviors, environment, illnesses and their causes, and health at birth and causes of death. Improving health begins by understanding these measures and this report is one of several projects that make up the HIP-Cuyahoga initiative.

Community Health Improvement Plan (CHIP)

This comprehensive report identifies and addresses significant health priorities and will serve as a roadmap to addressing the most important factors that affect health in our county.
